- Developing meal plan according to individual needs depending upon the sugar levels, weight and financial status
- Using food modesl for the better understanding of patients
- Providing educational materials, which are translated in Urdu for patient’s understanding
- Collecting baseline and follow up data on a patient’s dietary pattern, sugar levels and proposed meal plans using relevant electronic platform
- Consulting patients from DMP and Family medicine
- Assessing an individual's specific education needs, informed by the doctor’s checklist
- Identifying an individual's specific diabetes self-management goals
- Providing education and behavioral interventions, directed at helping patients achieve identified self-management goals
- Counseling patients with the aid of DMP educational material
- Counseling patients on Hyperglycemia and Hypoglycemia, using the Hypoglycemia and Hyperglycemia brochure
- Counseling patients regarding regular SMBG which includes:
- How to use glucometer
- How to record readings in SMBG booklet
- Counseling the patient regarding medications, which includes:
- Timings
- Compliance
Counseling the patient regarding Insulin which includes: - Myths related to insulin
- Removing the needle phobia by injecting needle to the patient
- Insulin injecting technique using Insulin Dose brochure, dose adjustment using Dose Adjustment brochure
- Proper site rotation using rotation guide for abdomen and thighs
- Timings
- Compliance
Providing patient with Foot Care Tips brochure if they have not already received it Documenting of the education-related data using DMP Diabetes Education form Ensuring all key aspects of education are covered by following the Diabetes Educator Checklist for New and Follow-up patients as a guide Conducting Diabetes Conversation Maps (DCM) Entering data
