GENERIC RESPONSIBILITIES
Provide basic and supportive patient care under the supervision of the Senior Staff Nurse/Staff Nurse.
Report in time on duty as per schedule shift in assign areas.
Measure, monitor, and accurately record vital signs and assigned patient observations.
Observe patients for changes in condition and promptly report signs of deterioration to the responsible nurse.
Assist nursing staff with routine nursing procedures within the defined scope of practice and competency.
Administer medications, authorized and permitted within the defined service scope.
Maintain accurate and timely documentation of patient care, monitoring, and interventions in HMIS-
Assist with patient admission, transfer, discharge, and transportation as required.
Communicate effectively with patients, families, nurses, physicians, and multidisciplinary team members.
Prepare medication requisitions, forward to pharmacy for dispensing and follow until its dispensed.
Evaluate each patient for their concern and required supplies, medication, labs, food and arrange at first hand.
Ensure appropriate availability and utilization of patient-care supplies and equipment.
Comply with IHHN policies, SOPS, clinical protocols, safety standards, and applicable regulatory requirements.
Assist in maintaining a clean, safe, and organized patient-care environment.
Follow infection prevention and control practices, including hand hygiene and appropriate use of PPE.
Maintain patient safety, dignity, privacy, confidentiality, and rights at all times.
Demonstrate professionalism, teamwork, accountability, compassion, and respect in all patient-care activities.
Participate in training, competency assessments, quality improvement, and accreditation activities.
Uphold IHHN's mission, values, ethics, and patient-centered care standards.
After fulfilling the general nursing responsibilities, the Certified Nursing Assistant shall be responsible for performing the following area-specific duties, based on the clinical area assigned:
A. CRITICAL CARE
Assist nursing staff in providing basic care to critically ill patients under supervision.
Monitor and report vital signs and other assigned observations.
Assist with positioning, hygiene, feeding, and patient comfort.
Support preparation and availability of critical care equipment and supplies.
Immediately report any deterioration or abnormal observation to the Staff Nurse.
Follow strict infection prevention and patient safety practices.
B. EMERGENCY MEDICINE
Assist with rapid patient preparation, assessment, and monitoring under supervision
Record and report vital signs and changes in patient condition promptly,
Assist with patient transfer, positioning, transportation, and basic emergency care.
Support Senior Staff Nurse/ Staff Nurses during resuscitation and emergency procedures within scope of practice.
Ensure availability of emergency supplies and equipment,
Assist with patient admission, transfer, and discharge processes.
C. INPATIENT
Assist patients with hygiene, feeding, mobility, positioning, and other activities of daily living.
Monitor and record vital signs and assigned patient observations.
Assist Senior Staff Nurse/ Staff Nurses with routine patient care and procedures within scope of practice.
Observe and promptly report changes in patient condition.
Support admission, transfer, and discharge activities.
Accompany physicians' during rounds in the unit and note order given during round & request doctor to write order in HMIS about any change in patient care management including medication.
Take shift handover bed to bed from incoming shift of the unit, related to patients, files, medication, investigation reports, intake & output chart etc. and pending tasks carry forward by outgoing shift to incoming shift & reasoning.
Document take over in HMIS and any significant concern to follow urgently.
D. LABOR & DELIVERY ROOM
Assist in providing basic supportive care to women during labour and the postpartum period.
Monitor and report assigned maternal observations and vital signs.
Assist with patient positioning, hygiene, comfort, and mobility.
Support the nursing team during delivery and related procedures.
Assist with immediate newborn care as directed and within competency.
E.
OPERATING & RECOVERY ROOMS
Assist in preparing the patient and operating room for procedures.
Support patient transfer, positioning, and comfort under supervision.
Assist with preparation and availability Of required supplies and equipment.
Assist with post-operative patient transfer and handover.
Support cleaning, organization, and replenishment Of the operating room as required.
Follow doctor pre-op order & ensure (consent signed, NPO, site preparation, pre-op bath, change of OR dress, arrangement of blood and Maintain 1/0, pre-op medication, check & record fresh vital signs) and transfer walking patient on wheel chair.
Follow Post-op order, last vital signs reading from OR, check surgery site dressing for bleeding, Cyanosis, I/V site,
surgical drain/any drain, maintain 1/0, and be watchful for change in vital signs/bleeding at surgery site.
F.
OUTPATIENT
Assist with patient registration, preparation, positioning, and basic assessment activities as assigned.
Monitor and record vital signs.
Assist nurses and physicians during consultations and minor procedures.
Administer authorized medications within scope of practice and organizational policy.
Provide basic patient instructions as directed by the nursing team.
Support patient flow, infection prevention, documentation, and maintenance of the clinical environment.
G. OTHER RESPONSIBILITIES
Perform any other task assigned by the immediate Supervisor that is commensurate with your knowledge and skills.
