Job Description

The Pharmacist is responsible for rational use of medicine including prescribing, dispensing and administration to assure patient safety, furthermore responsible for inventory management, departmental documentations and timely delivery of medicine to the patients as per regulatory requirement.


TASKS & ACCOUNTABILITIES

1. Medication Dispensing & Review

  • Ensure proper handing-over and taking-over Pharmacy charge in compliance with departmental policies and protocols.
  • Ensure timely dispensing and distribution of medications, reviewing patient profiles and Computerized Provider Order Entry (CPOE) systems to prevent drug interactions and promote rational drug use.
  • Contact prescribing physicians regarding medication order discrepancies and document interventions.
  • In cases of medication unavailability, contact physicians to switch to available alternatives/substitutes. If unavailable, process Non-Formulary Item (NFI) requests via duly signed NFI forms.
  • Ensure all medication orders are processed promptly and delivered to nursing units without delay.

2. Inventory & Storage Management (Operational)

  • Monitor inventory stock levels and assist in managing when needed.
  • Prepare and forward departmental requisitions to the warehouse.
  • Receive goods from the warehouse, verifying batch numbers, expiry dates, quantities, packaging integrity, and cold chain compliance (if applicable). Shelve stock in designated locations under specified temperature conditions.
  • Ensure appropriate storage of light-sensitive medications, cytotoxic drugs, high alert medications, and look-alike/sound-alike (LASA) drugs.
  • Ensure temperature monitoring, dusting, refrigerator cleaning, and related documentation are accurately
  • Ensure Planned Preventive Maintenance (PPM) for pharmaceutical refrigerators, air conditioning units and other location- specific equipment, and keep equipment calibration records.

3. Clinical & Specialized Area Support

  • Outpatient Department (OPD) Pharmacy: Counsel patients and address their queries during direct medication dispensing. Ensure accurate dispensing and management of specialized therapies (e.g., Insulin, Tofacitinib, Hep-C medicine, ATT therapy, etc.) within the OPD Pharmacy. Provide patient counselling on adherence to therapy and required lab monitoring (e.g., LFTs, RFTS, CBC, cardiac markers etc.), maintain manual/electronic records (registers/Excel Sheet), and support therapy tracking.
  • Emergency Room (ER) Pharmacy: Conduct bedside visits for direct patient care and drug utilization reviews.
  • Inpatient Pharmacy/ Satellite Pharmacy: Review requisitions for the right drug with dose, frequency, dosage form, route of administration, and document all interventions. Where applicable, ensure adherence to all sterile area Standard Operating Procedures (SOPs) and protocols.
  • Operating Rooms (OR) Pharmacy: Ensure adherence to all SOPs and protocols related to preparing supply baskets according to requested surgeries and clearing postings on HMIS of all manually issued supplies requested during urgent procedures for urgent need. Maintain and document temperature, humidity, and pressure differentials in sterile areas.

4. Drug Information & Documentation

  • Provide drug information to healthcare professionals as requested and maintain corresponding records.
  • Complete KPI and stock cover sheets during each shift.
